Introduction. Complex visual hallucinations with retention of insight due to impairment are key symptoms Charles Bonnet syndrome. The syndrome is a standalone diagnosis in ICD-11, but clinical praxis often misdiagnosed as psychosis or early stages dementia and goes underreported by patients because the fear being diagnosed mental illness. Case report. We present four cases elderly impaired vision who had retained referred for psychiatric consultation. All complex, vivid colorful consisted realistic objects, people, animals scenery that tend recur. Their emotional response impact on quality life differed, psychopharmacotherapy was recommended depending their psychological symptoms. Empathic explanation origin reassurance absence illness caregivers vital importance all cases. Conclusion. aging population increase consequence number emphasize multidisciplinary approach diagnostic procedures treatment. Increased awareness characteristic therapeutic approaches required among physicians contact and/or patients.
